USEA Rider Award Application Mail this form in after you have placed 1st - 5th at the same level at three events to receive an award.
USEA Volunteer Award Application

 Mail this form in after you have volunteered at at least 3 USEA events.

USEA II Points & Awards Registration Form

 Apply each year to be eligible for USEA II Rider Awards. Only $20/division before July 1.
